7 Common Reasons Claims Get Denied (and How to Prevent Them)

Every denied claim costs your practice twice: once in the work to rework it, and again in the delay to get paid. The good news is that most denials trace back to a short list of preventable causes.

1. Eligibility and registration errors

Wrong subscriber IDs, outdated insurance or typos in a patient name are among the most frequent causes. Verify coverage before the visit, not after the claim bounces.

2. Missing or invalid prior authorization

Many services require approval in advance. Build authorization checks into scheduling so nothing is performed without one.

3. Coding mismatches

Diagnosis codes that do not support the procedure, missing modifiers, or unbundled services all trigger rejections. Specialty-trained coders and claim scrubbing catch these before submission.

4. Timely filing misses

Each payer has a filing deadline. Track it per payer and work aging claims early so nothing expires unpaid.

5. Duplicate submissions

Resubmitting a claim that is still in process can lead to a duplicate denial. Check status before you resend.

6. Incomplete documentation

If the clinical note does not support the service billed, the claim will not hold up on review.

7. No denial tracking

If you do not record why claims are denied, the same errors repeat. Categorize every denial and review the patterns monthly.

The takeaway

Prevention beats rework. A clean front end, careful coding and consistent tracking will do more for your cash flow than chasing denials after the fact.
